St. Hubert Pastoral Care offers support for those grieving the death of a loved one, the loss of a marriage, and those in need of healing. We welcome help from people who feel called by God to serve the needs of others and the greater community in pastoral ministry.
We invite you to explore ways that we in pastoral ministry can assist you in your need, or enable you to serve the needs of others. Some of our current groups and ministries include:
Young at Heart
St. Hubert parishioners age 55 and older gather weekly on Thursday mornings at 8:00 AM for coffee and conversation. Join us for food, faith, fun and friends!
Homebound Communion Ministry
As members of Christ's body and by virtue of our baptism, we are all ministers of Christ's healing. Homebound visitation is one way in which we minister pastorally to those in our parish community who are unable to attend Sunday Mass.
The Extraordinary Ministers of Holy Communion visit the elderly, the sick and the homebound and bring them the Body of Christ in Holy Communion weekly. The ministers also visit with and listen to them. St. Hubert Catholic Community has trained ministers who will bring communion to your home on Tuesday mornings. The priest and pastoral minister are also available for visits upon request.

We Care Card Ministry
The card ministry group prepares handmade cards to give to parishioners for a variety of occasions including "thinking of you" for the homebound, those receiving sacraments, sympathy and one-year anniversary of the death of a loved one, prayer shawl cards, birthday cards for the Love INC Birthday Closet, and all occasion greeting card packets for the homebound.
Anyone can join us, even if you don't have experience. We provide all the materials and on-the-job training!

Prayer Shawl Ministry
A prayer shawl brings comfort to the sick and those who are grieving; it brings hope to those in despair. It is created with care, love and prayer by a group of parishioners of all ages and talents.
Finished prayer shawls are brought to the monthly meeting and group members pray a special blessing over them. The prayer shawl ministry will gladly make and give this expression of caring to you.

Prayer Chain Ministry
St. Hubert prayer chain consists of a group of people who pray individually in their homes for special prayer requests received by fellow parishioners. The power of prayer is tremendous! If you would like to join our prayer chain or have a special prayer request, contact the parish office at 952-934-9106.
Southwest Grief Coalition
If you are grieving the loss of a loved one - spouse, parent, sibling, child or friend - St. Hubert Catholic Community offers you the opportunity to join a support group for your journey of healing. St. Hubert is a member of the Southwest Grief Coalition based in Eden Prairie and Chanhassen. .
